AT Home Hospice’s team of physicians, nurses, hospice aides, social workers and chaplains believe in and follow the Golden Rule. We treat others the way we want to be treated. We are honored to help take care of your loved one’s needs during the last days of their life.
Skilled Nurses — Our nurses are licensed professionals who coordinate care and foster communication between members of the health team. Nurses provide the following:
- Patient assessment
- Medication management
- Pain management
- Symptom control
- Special treatments such as wound care
- Other services as outlined in the patient’s plan of care
Certified Nursing Assistants/Hospice Aides – Our hospice aides are certified caregivers who work very closely with families, patients and nurses to enhance your loved one’s care. Their services include but are not limited to:
- Personal care assistance
- Light housekeeping
- Life enhancement activities such as reading to patients, helping write letters, or going on walks
Medical Social Workers – Our medical social workers are licensed experts who provide social and emotional support, professional counseling, and assistance with obtaining needed financial and community resources.
Chaplains – AT Home Hospice cares for the whole person, including the spiritual needs of our patients and families. Chaplains assess the spiritual needs of patients and families, provide direct counseling, consult with community clergy members, and provide bereavement support.
Medical Director – Our Medical Director leads the hospice care team in developing individualized patient plans of care, provides consultation to other physicians regarding hospice care, and certifies terminal prognosis as part of the hospice eligibility process.
Other Services – AT Home Care provides other services such as speech therapy and rehabilitative services when appropriate. We also provide medical equipment, supplies, and medications related to the patient’s terminal condition.
