Personalized Care

AT Home Care offers personalized and comprehensive home care to individuals by a highly trained and dedicated staff of professionals. A Registered Nurse or Physical Therapist will conduct a preliminary evaluation of the types of services needed in your home. A team of professionals, together with family members, physicians and other health care professionals, then work to develop a care plan.

Transitional Care Program

AT Home Care provides family members and loved ones with several levels of attentive and compassionate care services in the home setting. Our Transitional Program naturally allows patients to progress from restorative care into palliative care, while ensuring continuity of services within the same organization. The patient can smoothly move from one care type to another when circumstances dictate.

Orthopedic Care Program

Our team of specialists brings advanced experience and the latest technology to the physical therapy treatment for each patient. We integrate the patient’s home and community concerns as well as their occupation, hobbies and interests to achieve the highest level of function.

Other Services

Many of these services including medications, medical equipment and supplies may be covered 100% if they are related to the hospice diagnosis.

Caring for Our Patients Is Our Privilege

Several years ago, my father’s doctor recommended hospice, and I was clueless at the time. My mom passed away years prior from cancer and Dad depended on me. I was his daughter and caregiver.

When I sat down with hospice and discovered the benefits available for my father and all who qualify, I was overcome with gratitude.

I’m thankful for the team who lovingly cared for my dad and kept me from falling apart. When my father’s journey ended, I decided to pursue hospice as a career. I thank God for directing my path…. After 7 years, hospice has proven to be challenging, yet beautifully rewarding.

I have one last closing thought: hospice is often misunderstood. Although you may have a diagnosis of 6 months or less, no human being knows your expiration date. I’ve had patients who survived over 2 years, giving them the gift of added time with their family. Some have even graduated from hospice due to their health improving. It happens!

The care you receive from our hospice staff is exemplary, and if you qualify, everything pertaining to your diagnosis is covered due to the benefits you have earned throughout your life. Hospice truly is a blessing and one I will be grateful for as long as I live.

Brightspring Hospice Foundation

The BrightSpring Hospice Foundation provides hospice patients and their families with essential non-hospice support for which they are unable to otherwise afford.

The foundation supports patients and families with their hospice journey. While the foundation does not provide health care services to patients, it does provide financial support to families in need of non-health care-related assistance.

We find comfort in knowing that all individuals can receive assistance beyond personalized and uplifting hospice care.
