AT Home cares for the terminally ill and helps patients live their final days to the fullest, as free as possible from disabling pain and mental anguish.
Our physicians actively participate in admission and eligibility decisions, as well as in-home visits for hospice patients.
In-home visits are performed by experienced and caring nurses. We are available 24 hours a day, 7 days a week in availability and support.

Treating the whole person includes helping to meet the spiritual needs of the patient and family. A chaplain treats the whole person — mind, body, and spirit. While assessing the patient’s and family’s spiritual needs, they provide direct counseling, consult with community clergy, and provide bereavement support.
The Medical Director leads the hospice care team in developing a plan of care to meet each patient’s specific needs, provides consultation to physicians regarding hospice care, and certifies terminal prognosis as part of the hospice eligibility process.
Music Therapy – Music is used throughout our lives to enrich our experiences. Music therapy is often used in the same way for people facing the last days of their lives. Music therapists are valued members of the Premier Hospice care team. They are professionally trained musicians who entertain patients and families. Whether it’s writing a song, learning to play guitar, or getting acquainted with the beautiful nuances of sound, music therapy can create a relaxing atmosphere and/or become a fulfilling diversion for a hospice patient. Our therapists work closely with each patient, helping them set and achieve musical goals.
Pet Therapy – There is a unique relationship that develops between a person and their pet. It could be because pets are loyal, affectionate, and comforting, or it could simply be that they are good listeners. Many people who receive end-of-life care appreciate the comfort of having one of our specially trained pets (especially dogs) by their sides. Pet therapy is a way for people to experience joy – an emotion that isn’t often associated with a life-limiting illness.
Volunteers are specially trained to support you and your family through companionship, grief support, and caregiver relief.
AT Home provides the level of care that is most appropriate to meet your individual needs.
Provided to individuals in the comfort of their home, whether the home is a private residence, a long term care facility, or an assisting living community.
Short term inpatient care at a hospice-approved facility. Patients receive intense palliative treatments to address pain and symptom management. Once symptoms are controlled, the patient returns to their home.
This level of care is medically necessary care for patients who may be experiencing a medical crisis requiring more intense pain or symptom management for short term, round the clock support.
As needed for family members or caregiver relief up to 5 days on an occasional basis at a hospice approved facility.
