Hospice Care

AT Home Care & Hospice uses a team approach to assure continuity of care thoughout the hospice process.

We will tailor an experienced team to meet your individual needs and deliver a personalized, focused care plan.

One in three Americans is caring for a loved one with a serious illness. They find themselves looking for answers to important questions and making challenging decisions every day. It’s not something anyone should have to go through alone. AT Hospice is there to help families when they need it most. 

Our services include:

  • On-Call Hospice Support – 24 hours a day, 7 days a week
  • Pain Management – Treatments to help with pain and discomfort
  • Symptom Management – Treatments to aid with anxiety, breathing, nausea and other symptoms
  • Medication Coordination – Allows proper medication to be available when needed
  • Registered Hospice Nurses Visits – Scheduled to meet your individualized hospice nursing care plan
  • Nurse Aide Visits – Scheduled to aid with  personal care: bathing, feeding, dressing and other duties
  • Hospice Team Visits – Hospice counseling, program implementation, insurance coverage, support and follow-up visits
  • Volunteer Visits – Scheduled for companionship, errands and light duties
  • Bereavement Services – 13-month bereavement services post-death to all family members and loved ones

Hospice Services

AT Home cares for the terminally ill and helps patients live their final days to the fullest, as free as possible from disabling pain and mental anguish.

Physician Services

Our physicians actively participate in admission and eligibility decisions, as well as in-home visits for hospice patients.

Nursing Care, Pain & Symptom Management, and Medication Coordination

In-home visits are performed by experienced and caring nurses. We are available 24 hours a day, 7 days a week in availability and support.

Bereavement Program

A program supervised by a qualified bereavement coordinator which offers support and services.

Social and Spiritual Services

Counseling for our patients and families. We help you through the emotional, financial, and spiritual process.

Hospice Volunteer Services

Volunteers visit you in your home and provide errands and companionship.

Pharmacy, Medical Equipment & Medical Supplies

We deliver and set up everything needed, including hospital beds, oxygen tanks, and wheelchairs.

Routine, Respite, General Inpatient and Continuous Care

AT Home Care & Hospice offers different types of care for your loved one, including short-term Respite Care, General Inpatient Care provided in a Medicare-approved SNF, and Continuous Care provided in the home.

Your Care Team

Our team of physicians, nurses, hospice aides, social workers, chaplains, and volunteers believe in the Golden Rule. We treat others the way we want to be treated. We help take care of your loved one’s needs during the last days of their life.

Licensed experts who adhere to the doctor’s plan of care. Services include:

  • Consistent, thorough care
  • Monitoring vital signs
  • Medication oversight
  • Symptom control
  • Wound care
  • Pain and symptom management

Certified professionals who work very closely with our nurses to ensure there are not any gaps in your loved one’s care. Services include:

  • Assisting in personal care
  • Light housekeeping
  • Life enhancement activities such as reading to the patient, writing letters, and going for walks.

Licensed experts who go beyond traditional health care so your loved one is connected to all the resources they need to be as comfortable as possible. Services include:

  • Social and emotional support
  • Assisting with community resource scheduling

Treating the whole person includes helping to meet the spiritual needs of the patient and family. A chaplain treats the whole person — mind, body, and spirit. While assessing the patient’s and family’s spiritual needs, they provide direct counseling, consult with community clergy, and provide bereavement support.

The Medical Director leads the hospice care team in developing a plan of care to meet each patient’s specific needs, provides consultation to physicians regarding hospice care, and certifies terminal prognosis as part of the hospice eligibility process.

Music Therapy – Music is used throughout our lives to enrich our experiences. Music therapy is often used in the same way for people facing the last days of their lives. Music therapists are valued members of the Premier Hospice care team. They are professionally trained musicians who entertain patients and families. Whether it’s writing a song, learning to play guitar, or getting acquainted with the beautiful nuances of sound, music therapy can create a relaxing atmosphere and/or become a fulfilling diversion for a hospice patient. Our therapists work closely with each patient, helping them set and achieve musical goals.

Pet Therapy – There is a unique relationship that develops between a person and their pet. It could be because pets are loyal, affectionate, and comforting, or it could simply be that they are good listeners. Many people who receive end-of-life care appreciate the comfort of having one of our specially trained pets (especially dogs) by their sides. Pet therapy is a way for people to experience joy – an emotion that isn’t often associated with a life-limiting illness.

Volunteers are specially trained to support you and your family through companionship, grief support, and caregiver relief.
